1. By completing the ILMF, you will confirm that you share the goals and targets of the Keydata/SLS/LCS Action Group ("KSLIT") and commit to making a 2% contribution to a KSLIT "fighting fund".
2. The ILMF should not take any longer than 5 minutes to complete, although if you used an IFA or other intermediary to invest you will need their contact details to hand.

4. Please download and complete the attached ILMF and pay the 2% Contribution as soon as possible. Both completion of an ILMF and payment of the 2% contribution are necessary in order to become a member. Registration alone is not sufficient.

This form relates ONLY to investments in the Keydata Secure Income Bond issues 1, 2 or 3, or bonds issued by SLS Capital SA or Life Settlements Capital SA (together the "KSLIT Bond Schemes"). For the avoidance of doubt, this form does NOT relate to investments in bonds issued by Lifemark, i.e. it does NOT relate to investments in Secure Income Bond 4; Defined Income Plans 1-8; Secure Income Plans 1-12 and 14; and some similar products with other names to suit special marketing needs.
